Laboratory Registration

Enter your labs details here. Once completed your registration will need to be approved by us. You will receive a confirmation email once your account has been activated.

All billing related emails will be sent to the lab coordinator, however you can specify an additional email address to be included in the notifications.

Add any additional billing information you would like printed on the invoices for this lab.

Co-ordinator

  • Will be automatically registered as a scorer upon successful lab registration
  • Will have access the entire labs results
  • Can appoint a scorer as a manager (giving them access to the entire labs results)
  • Is required to maintain the records of training status for each of the scorers
  • Will act as the point of contact for QSleep communication
  • Can nominate another scorer as the co-ordinator
